Committee Approval Form (PDF) -You will need 3 copies of this form, two printed and one electronically inserted as the first page of your thesis or dissertation. The first two copies should be printed and signed by your committee. The third copy (the electronic version) should have your committee members' names typed (you can do this on the screen). Do not insert the electronic copy with signatures on it.

ETD Embargo Form - This form should be turned in to the Graduate Office per the last step of the Graduation Process.

Thesis & Dissertation Publications Payment Form - Publication payment form for publication of your Thesis/Dissertation to Proquest/UMI. Note that publication to Proquest/UMI is mandatory for dissertations. This form should also be turned in to the Graduate Office per the last step of the Graduation Process.
